Abstract

Frozen shoulder (adhesive capsulitis) is a common musculoskeletal disorder

characterized by chronic pain and progressive limitation of both active and passive

shoulder range of motion, leading to functional impairment. This case report describes

a 70-year-old male with a history of type 2 diabetes mellitus who presented with left

shoulder pain and restricted movement for six months. The pain was severe (VAS 9) and

aggravated by overhead activities, accompanied by significant limitation in daily

functions. Physical examination revealed tenderness, muscle spasm, and decreased

range of motion, with a positive Apley test. The diagnosis was established clinically

based on typical findings. The presence of diabetes mellitus was identified as a major

risk factor contributing to disease severity and prolonged course. Management included

a combination of non-pharmacological and pharmacological approaches, such as

physiotherapy modalities (TENS, ultrasound, joint mobilization, and therapeutic

exercises), activity modification, nonsteroidal anti-inflammatory drugs, and

corticosteroids. Patient education and glycemic control were also emphasized. This

comprehensive management aimed to reduce pain, improve joint mobility, and restore

functional ability. Frozen shoulder requires early recognition and appropriate

intervention to prevent long-term disability, although recovery may take several months

to years, particularly in patients with comorbid conditions.
